Diskette Request Form

NOTE: This form is submitted to Ira through an email sent by your system and will not work if you use a web based email system (such as GMAIL). Sorry – I am not familiar enough with advanced web programming languages to have this form submit itself.
Your computer may require that you click on confirming boxes, as well as actually clicking send in your email program. In fact, it is VERY likely that the form will simply open up your email program and set up an email, but not send it.
Please make sure an email is actually sent (by checking your sent mail), or it is likely it wasn’t.
NOTE: You cannot copy/paste this page into an email – I will get only blank boxes, even if you filled them out.
FINAL NOTE: Because too many people who requested disks did not donate, even to cover costs of postage, I no longer will advance the money to ship outside the United States. However, for those lucky people in Australia or New Zealand, please contact Ian Mavric who will be fulfilling disk requests in those countries. The email to use is sixpointninemb@hotmail.com. Do NOT use this form.
Name and Email:
Full Mailing Address (NOTE: I will NOT ship to P.O. Boxes). Effective 2/17/2011 I will no longer ship outside the United States; too many people have requested disks and not reimbursed the almost $20 in postage it costs me to ship them. For those living in Australia or New Zealand, you may SEPARATELY contact Ian Mavric at sixpointninemb@hotmail.com who will be fulfilling disk requests in those countries.

What TRS-80 Model(s) do you have?
What DOS’s do you need?
Model I
Model III
Model 4 (*)
Model 4P (**)
* NOTE: A Model 4 is also a Model III, and can boot Model 3 DOS’s. To use Model III Mode (and Model I and Model III software), you MUST also use Model III DOS disks as well.
** NOTE: A Model 4P is also a Model 4, and can boot Model 4 DOS’s. The disks listed under Model 4P are specialty disks ONLY for the Model 4P. If you have a 4P you should be requesting Model 4 DOS’s as well.
Check if Applicable:
I understand that there are costs involved in the creation, packing and shipping of the disks I am asking for, and I intend to donate once I receive the disks.
Test Your Drives FIRST:
To test your drives on a Model I please type the following from Level II BASIC:
POKE 14304,1
and look at drive 0. If Drive 0 is on/spinning, it is at least hooked up properly.
As an aside, POKE 14304,2 should turn on drive 1, but that is not vital for purposes of a boot disk.
To test your drives on a Model III and 4 please type the following from Level II BASIC:
OUT 244,1
and look at drive 0. If Drive 0 is on/spinning, it is at least hooked up properly.
As an aside, OUT 244,2 should turn on drive 1, but that is not vital for purposes of a boot disk.
Check only if your drive lit up.
